See how Illinois public health and community health nurse experts collaborate with academic nursing faculty to advance the profession of nursing to help our nation create a culture of health, reduce health disparities, and improve the health and well-being of Illinois communities.

Nurse experts will share the successes and lessons learned through academic-practice collaboration in building and sustaining partnerships.  Projects include the use of social media in marketing health improvement campaigns.  The use of social media in evaluating campaign effectiveness.  Interdisciplinary collaboration – providing education throughout a rural county.  These projects are supported by a Robert Wood Johnson Foundation (RWJF) grant.

The webinar will include the role of nurses in improving the health of individuals, families, and communities by addressing social determinants of health and providing effective, efficient, equitable, and accessible care across the care continuum.  Presentations will also identify the system facilitators and barriers to achieving the goals.  The partnerships showcase the ability of nurses to serve as change agents in creating systems that bridge the delivery of health care and social needs care in the community.
