Step 3. Locate the evaluation form below the video window. Answer membership enrollment questions, evaluate the activity, and create your certificate of completion with 1.0 CE credit. Use the scroll bar on the right side of the form to view all questions.

Step 4. At the end of the form, click “Submit”

All enrollment steps must be completed by June 30, 2026, at 11:59 PM. Your membership begins August 1st, 2026, giving you access to all ANA-Illinois and ANA benefits!
